Oxidative stress is an imbalance between free radicals and antioxidants in your body. Free radicals are oxygen-containing molecules with an uneven number of electrons. The uneven number allows them to easily react with other molecules. Free radicals can cause large chain chemical reactions in your body because they react so easily with other molecules. These reactions are called oxidation. They can be beneficial or harmful.

Nrf2 is a powerful protein that is latent within each cell in the body, unable to move or operate until it is released by an Nrf2 activator. Once released it migrates into the cell nucleus and bonds to the DNA at the location of the Antioxidant Response Element (ARE) or also called hARE (Human Antioxidant Response Element) which is the master regulator of the total antioxidant system that is available in ALL human cells. This balancing act can become disrupted as one age, or if the body is overcome by malfunction and disease resulting in further cellular damage. This damage is known as oxidative stress and is accompanied by free radical production. Nrf2 Activators as Attractive Therapeutics for Diabetic Nephropathy

Diabetes is the major cause of chronic kidney disease worldwide (1) with treatment options focused primarily on glucose control, blood pressure, lipid lowering, and the blockade of the renin-angiotensin system (2). However, despite intensive metabolic control and other interventions (3), the unrelenting decline in kidney function means that for many patients the condition progresses to overt kidney failure. This underpins the urgent need for novel approaches to manage the ever-increasing number of patients with diabetes and chronic kidney disease.


One approach that is attracting attention is the use of compounds to bolster the natural cytoprotective responses of the body. The transcription factor NF-E2–related factor 2 (Nrf2), together with its negative regulator, Kelch-like ECH-associated protein 1 (Keap1), is considered one of the most important cellular defense mechanisms to combat oxidative stress (4) with a particular role in the regulation of phase II detoxifying enzymes (Fig. 1). In particular, NADPH quinone oxidoreductase, glutathione S-transferase, heme oxygenase-1, and γ-glutamylcysteine synthetase are well-studied targets of Nrf2 that are upregulated through the antioxidant response element found in the promoters of these genes (5). Therefore, the coordinated upregulation of genes coding for detoxification, antioxidant, and anti-inflammatory regulators is seen as a potential therapeutic strategy to protect against insults such as inflammation and oxidative stress that are known to be enhanced by the diabetic milieu.


It is therefore not surprising that attention has focused on identifying small molecule activators of the Nrf2/Keap1 pathway. Many chemically diverse activators have already been identified (6), including the glutathione peroxidase-1 mimetic ebselen (7), sulforaphane found in cruciferous vegetables (8), caffeic acid phenethylester from the bee product propolis (9), cinnamic aldehyde (found in cinnamon bark), and most recently, bardoxolone methyl (10,11). Many have shown promising actions relevant to diabetes complications.

‘Telomeres’ Role in Aging

Telomeres in human blood are extremely important to aging. At birth, they tend to be around 8,000 base pairs long, around 35 years they are 3,000 base pairs long and by age 65 they can be less than 1,500 base pairs long. Cells usually divide between 50 and 70 times in a lifetime and during each division 20 to 300 telomere base pairs are lost. Telomere length varies from person to person at any given age. When studied, subjects aged 65 who had less telomere base pairs than other subjects of the same age were more likely to die of heart disease or cancer.

For instance, one study found that women with chronically ill children had dramatically shorter telomere chains than those of women with low-stress children. Certain components of the shiitake mushroom have hypolipidaemic (fat-reducing) effects, such as eritadenine and b-glucan, a soluble dietary fiber that’s also found in barley, rye, and oats. Studies have reported that b-glucan can increase satiety, reduce food intake, delay nutrition absorption and reduce plasma lipid (fat) levels. A 2011 study published in the Journal of Obesity examined the effects of shiitake mushrooms on plasma lipid profiles, fat dispositions, energy efficiency, and body fat index. Rats were fed a high-fat diet for a six-week period. Researchers found significant effects of dietary intervention on body weight gain. Rats on a high dose of shiitake mushroom diet (which involved adding mushroom powder to a high-fat diet) had 35 percent lower body weight gains than rats on low and medium shiitake mushroom diets. Rats on the high dose shiitake mushroom diet also had significantly lower total fat masses and had a trend of lower fat accumulation. The researchers concluded by suggesting that shiitake mushrooms can help prevent body weight gain, fat deposition, and plasma triacylglycerol when added to a high-fat diet. This encourages an effort to pursue human studies that examine the efficacy of shiitake mushrooms for the prevention and treatment of obesity and related metabolic disorders.

Mushrooms have the ability to boost the immune system and combat many diseases by way of providing important vitamins, minerals and enzymes. A 2015 study published in the Journal of the American College of Nutrition evaluated 52 healthy males and females, aged 21–41 years, to determine if shiitake mushrooms could improve human immune function. The study involved a four-week parallel group trial that involved participants consuming either five or 10 grams of mushrooms daily. The results suggest that consuming mushrooms improved cell effector function and improved gut immunity. There was also a reduction of inflammation due to mushroom consumption.

Research suggests that shiitake mushrooms help fight cancer cells and the lentinan in shiitakes help heal chromosome damage caused by anticancer treatments. This suggests shiitakes could be used as a potential natural cancer treatment. A 2006 study published in the Journal of Alternative and Complementary Medicine investigated the potential roles of an ethyl acetate fraction from shiitake mushrooms. The study involved two human breast carcinoma cell lines, one human nonmalignant breast epithelial cell line and two myeloma cell lines. The results suggest that shiitake mushrooms were able to inhibit growth in tumor cells with their mycochemical value. Shiitake mushrooms have sterol compounds that interfere with the production of cholesterol in the liver. A 2011 study at the UCL Eastman Dental Institute in London tested shiitake’s antimicrobial effects on gingivitis, a preventable disease that involves inflammation of the gums due to the buildup of a microbial biofilm at the gingival margin. The effectiveness of shiitake mushrooms was compared to that of the active component in the leading gingivitis mouthwash, containing chlorhexidine.

The total bacterial numbers, as well as the numbers of eight key organisms in the oral community, were investigated after treatment. The results indicated that shiitake mushroom extract lowered the numbers of some pathogenic organisms without affecting the organisms associated with health, unlike chlorhexidine, which had a limited effect on all organisms
